BloodbornePathogenExposureControlPlan
37-page plan · 13 sections + 2 appendices · plus fill-in checklist, how-to guide and sources · 64 pages across 4 files
Somebody sticks themselves at four on a Friday and nobody in the building knows what happens next.
37 pages · 581 fill-in fields · fill-in PDF
Covers every US stateFederal baseline. The same across every US state.
Check what you already own. The Hazard Communication Program covers the chemicals on the shelf. This covers blood. They are different standards with different duties, and a studio that breaks skin and uses disinfectants is under both.

Blank business paperwork: fillable PDF forms a business prints or types into. It contains no medical, health or clinical advice, gives no treatment guidance, and states no opinion about any person's condition or care. The practitioner or business owner supplies all clinical content. A complete fill-in written exposure control plan for a medspa, tattoo or piercing studio, permanent makeup artist, or small clinic. It is built to the employer duties in the federal OSHA standard at 29 CFR 1910.1030, and every requirement it states is quoted from the published text of that standard, read on 30 August 2026, with the paragraph number next to it. Inside: the exposure determination by job classification and task, the engineering and work practice controls, PPE, the written cleaning schedule and regulated waste, the hepatitis B vaccination offer with the mandatory declination statement reproduced word for word from the standard's own appendix, the post-exposure incident report and follow-up log, the fourteen training elements, the sharps injury log, and the annual review the rule requires by name. 581 fill-in blanks across 13 sections, plus a one-page emergency summary to pin up by the sharps container. This is the federal baseline. 22 states run their own state safety programs for private employers and those can be stricter, so check your own state before you rely on it.

What lands in your inbox
Four files. Two of them do the work.
You fill in the document itself and hand it on. The checklist is there so you do not leave a blank in it. Everything else is reference. Every file is a PDF you can type into on screen, or print and write on, so you need nothing else to use it.

Who needs it
A medspa, tattoo studio, piercing shop or permanent makeup artist with employees who break skin
A small clinic whose staff may contact blood as part of the job
An employer who has been asked for a written exposure control plan by an insurer, a landlord or an inspector
A studio whose plan was written years ago and has never had its annual review
An owner who has never offered the hepatitis B vaccination and does not know the deadline for doing so
Who does not
A workplace where nobody has a reasonably anticipated contact with blood as part of their duties.
An HIV or HBV research laboratory or production facility. That is a separate paragraph of the standard which this plan does not reproduce or answer.
A sole operator with no employees, because the standard is about protecting employees.
An employer who needs a hazard communication program for chemical products. That is a separate standard and a separate document.
A practice that already runs a reviewed plan with a current exposure determination, an annual training record and a sharps injury log.
Somebody who believes a signed waiver removes responsibility. In many states it does not, and this pack says so.
The rest of the set
One document is rarely the whole job.
Businesses like yours usually start with these. It is where a trade starts, not a complete legal inventory of everything it could ever be asked for — no list on any website is that. Every line below says where it came from.
Showing what a tattoo studio / piercing shop is asked to carry.

Tattoo studio / piercing shopEleven items on the list, one of them a multi-file kit rather than a single document. You are looking at one of them. We sell six of the other ten; four we do not have yet.

Your state can add to this. We have written down three places where this trade’s set differs from one state to the next. What we sell is written to the federal baseline, and a state that runs its own programme can ask for more than the federal rule does.

HIPAA notice of privacy practices & authorisations
01The plan itself, 13 sections with 581 fill-in fields, as a fill-in PDF you can type into
02An exposure determination laid out the way the rule asks for it: all-exposed classifications, some-exposed classifications, and the task list that decides who in the second group is covered
03The hepatitis B vaccination offer and record, with the mandatory declination statement reproduced word for word from the standard's own appendix
04A 24-field exposure incident report, a follow-up log, and a confidential sharps injury log that carries no names by design
05The fourteen training elements the rule requires, printed in full, with a 12-row training record
06The annual review page and the safer-devices consultation the rule requires you to document, which is the paragraph small employers most often miss
07A one-page emergency summary to pin up by the sharps container, and a fill-in checklist listing every one of the 581 blanks by section
Plain-English Monday actions
What to do first
1Fill Section 3 with your staff list open. The determination is made as if nobody wore gloves, which is what the rule says.
2Print Section 9 and put it on the wall where the sharps container is
3Arrange the hepatitis B vaccination offer before your next hire starts, because it has a ten working day clock on it
4Ask the people who hold the needles which safer devices they want, and write down that you asked
5Put a date in the diary for the annual review, because this is the one standard that names its own interval

This is the federal OSHA baseline only. 22 states run their own state safety programs for private employers and those can be stricter. You must check your state before you rely on it.
Not medical advice. What is done for an exposed person is decided by a licensed healthcare professional, and this document neither makes nor influences that decision.
No promise of a regulatory outcome. OSHA does not review or endorse written plans, and no template can promise a result.
Paragraph (e) of the standard, HIV and HBV research laboratories and production facilities, is not reproduced or answered.
No hazard communication program, no personal protective equipment program, no respiratory protection program, no sterilisation or instrument reprocessing procedure. Each is separate and several reach the same workplace.
No state or local rules on regulated medical waste disposal, body art licensing or infection control inspection. Those vary by state and often by county.
No training delivered. The plan records who was trained and by whom; it does not train them.
Not legal advice and not safety consulting advice. It is a document you fill in and own.
No Spanish translation in v1.0.
Before you buy
The terms, in the words we would use out loud.
Updates are free, forever
When a rule changes, or an objection to this plan keeps coming up, we rewrite it and the new version lands in your account. You do not pay again. That applies to everyone who buys, member or not, for as long as we sell this document.
No refunds for a change of mind
The files are yours the moment payment clears, so we do not give money back just because you no longer want them. You agree to that in the box above the button before you pay, not afterwards in small print.
Broken or wrong, we fix it free — for everyone
If a file is the wrong one, will not open, is missing pages, or does not match what this page says, that is on us and we correct it at no cost, member or not. A version fitted to your business is either included with membership or $49 on its own. That promise is the reason the no-refund rule is fair.
A version fitted to your business: $49 once, or $30/month
Want this plan edited to match how your business actually works? A non-member buys one tailored edit for $49, no membership needed, and it comes back within 2 business days. A Simply Current membership is $30/month and every fitted edit is included — it wins on the second edit.
One more time, plainly: this is United States · federal OSHA baseline · general industry. A state or a city can be stricter than the federal floor, so check your own before you file or submit anything built from it. Anything we add for your state ships as a free update.
