Blank in every state — your state's law decidesNot scoped to any state. This is a blank form, and the law that decides whether what you write into it holds is your own state's, often your city's. Every state-specific figure in it is left blank for you to fill from your own.
Check what you already own. The GC Bid and Change Order bundle is what you send an owner. This is what you sign with a trade. They meet in one place only, the change-order mechanics, because the same change has to travel in both directions or somebody eats it.

The agreement a general contractor signs with a specialty trade on a job: a 40-line scope decided one line at a time, 24 exclusions initialled at the pre-construction meeting, schedule and payment mechanics including a schedule of values and a payment application, a change-order form and log, an insurance section written so a certificate gets checked rather than filed, an indemnification page, a 26-line safety acknowledgement signed by somebody who will be on the site, and four lien-waiver templates. 1,089 fill-in blanks across 17 sections. It does not decide whether anybody is an employee or an independent contractor, because the agencies test the facts rather than the label, and it warns that several states mandate their own waiver form. It cites no regulation.

Four files. Two of them do the work.
You fill in the document itself and hand it on. The checklist is there so you do not leave a blank in it. Everything else is reference. Every file is a PDF you can type into on screen, or print and write on, so you need nothing else to use it.

Who needs it
A general contractor whose subcontracts are a purchase order with a price on it
Anybody who has let a crew start and then discovered the insurance had lapsed
Somebody arguing at the end of a job about extra work that was agreed on a nod
A contractor who has signed a generic waiver in a state that prescribes its own form
Anyone who has had a backcharge appear on a payment application with no warning
Who does not
Anybody wanting a bid to send an owner. That is the GC bid and change-order bundle, and this is the other side of the same job.
Anyone expecting the agreement to settle whether a worker is an employee. The agencies test the reality of the relationship, and this document records what the parties believe rather than declaring it.
Somebody who needs the lien-waiver form their state prescribes. Several states mandate one, and the four here are a working shape rather than that form.
Anyone wanting a ready-made indemnity clause. Several states restrict indemnity in construction contracts, differently, and that page is questions with a flagged warning.
Anybody looking for retainage percentages, payment deadlines, lien deadlines or insurance limits. Every one is a state answer and every one is a blank.
Somebody who believes a signed waiver removes responsibility. In many states it does not, and this pack says so.
The rest of the set
One document is rarely the whole job.
Businesses like yours usually start with these. It is where a trade starts, not a complete legal inventory of everything it could ever be asked for — no list on any website is that. Every line below says where it came from.

1Fill the rules page once for the state you work in, from a lawyer and your broker
2Have the indemnity wording written once, locally, and reuse it on every subcontract
3Diarise the earliest insurance expiry on every live subcontract you already hold
4Print the safety page and get it signed by a person who is actually on the site
5Find out which lien-waiver form your state requires before the next payment application
Questions
What people ask before they buy it.
Who is this agreement written for?
A general contractor signing up a specialty trade on a job. It carries a 40-line scope decided one line at a time, 24 exclusions the subcontractor initials at the pre-construction meeting, a schedule of values and payment application, a change-order form and log, and a pre-construction meeting agenda. 1,089 fill-in blanks across 17 sections.
How does it handle the certificate of insurance?
Section 7 is written to be checked rather than filed. It records who checked the certificate, what was missing, whether the endorsement itself was produced, the date the earliest policy expires and who diarises it, and what happens to payment if cover lapses. It states no limits and no policy wording, because those are set by your insurer and the contract above you.
Are the lien waivers valid in my state?
The four waiver templates are a working shape, not a state-mandated form, and the document says so in capitals. Several states, including California, Texas and Florida, prescribe the wording of the waiver itself. Section 13.1 records whether your state mandates a form, its exact name, and who confirmed it is current.
Does it decide whether my subcontractor is an employee?
No. Section 1.2 explains that the question is decided by the facts of the relationship, by agencies and courts, not by the sentence in the contract. The agreement records the question and leaves it to the facts.
Official-source registry
Named before release.
These are the real rules this plan is built on. Every citation was read against the published text on the date shown above, and each link goes to the official source, not to a summary.
No retainage percentage, prompt-payment deadline, lien deadline, preliminary notice period or notice form, for anywhere.
No insurance limit, policy form, endorsement wording or additional-insured language.
No indemnity clause and no limitation of liability.
No statement about whether any state permits pay-when-paid or pay-if-paid terms.
No licence threshold and no statement about who needs a licence.
No safety standard, training requirement, inspection interval or equipment specification. The acknowledgement records what the subcontractor confirms and nothing more.
No wage determination, public-works requirement or payroll-reporting procedure.
No decision about worker classification. The label on a contract does not settle it.
No prices, rates, markups or allowances. Every one is a blank.
Not legal advice. Have a lawyer in the state the job is in read the finished agreement once.
Before you buy
The terms, in the words we would use out loud.
Updates are free, forever
When the law behind this agreement moves, or a clause keeps causing an argument, we rewrite it and the new version lands in your account. You do not pay again. That applies to everyone who buys, member or not, for as long as we sell this document.
No refunds for a change of mind
The files are yours the moment payment clears, so we do not give money back just because you no longer want them. You agree to that in the box above the button before you pay, not afterwards in small print.
Broken or wrong, we fix it free — for everyone
If a file is the wrong one, will not open, is missing pages, or does not match what this page says, that is on us and we correct it at no cost, member or not. A version fitted to your business is either included with membership or $49 on its own. That promise is the reason the no-refund rule is fair.
A version fitted to your business: $49 once, or $30/month
Want this agreement edited to match how your business actually works? A non-member buys one tailored edit for $49, no membership needed, and it comes back within 2 business days. A Simply Current membership is $30/month and every fitted edit is included — it wins on the second edit.
One more time, plainly: this is a blank agreement and it is scoped to no state. Every state-specific figure in it is left blank for you to fill from your own, and whether what you write in holds is decided by your state's law, often your city's. Check yours before you rely on it.
Simply Digital sells blank business documents. It is not a law firm, this is not legal advice, and nothing here creates an attorney-client relationship. Have a lawyer licensed in your state read the finished document once.
